Job Details:

Cape Cod Healthcare is the leading provider of healthcare services for residents and visitors of Cape Cod.  With more than 450 physicians, 4,700 employees, and 1,100 volunteers, Cape Cod Healthcare operates two-acute care hospitals, the Cape's leading provider of homecare and hospice services (VNA), a skilled nursing and rehabilitation facility (JML Care Center), an assisted living facility (Heritage at Falmouth), the Cape's only local laboratory service (CCHC Laboratory Services) and numerous health programs.  We are currently seeking a dedicated Admissions Coordinator to join our team at Cape Cod Hospital.



The Admissions Coordinator is responsible for facilitating admissions to the inpatient psychiatric unit, Partial Hospital Program (PHP), or other facilities to ensure that every step of the admission process is planned and executed. 



1.     Working closely with MDs and Allied Health Professionals (AHP) in Observation Purple and Cape Cod Hospital, ensures that the appropriate level of care is met for patients in an efficient and timely manner.

2.     Contacts managed care representatives, third party payers, primary care physicians or other authorized persons in order to provide clinical and/or demographic information required to verify insurance and/or obtain authorization for clinical services.

3.     Obtains necessary insurance verifications and authorizations for Inpatient Admissions and Partial Hospital Program.

4.     Triages incoming calls from either internal or external sources.

5.     Places calls to outside facilities when the inpatient unit or PHP does not have available beds or the patient does not meet level of care.

6.     Logs daily tracking of referrals including number of admissions, discharges and consults.

7.     Assists referral sources and/or patients with alternative community resources.

8.     Appropriately handles all non-intake calls from the community seeking information, assistance, outpatient services, and/or detox services which come through the access line.

9.     Utilizes on-line MassRevs system to verify up-to-date, accurate insurance information.

10.  Maintains knowledge of CMS, DPH and DMH standards.

11.  Participates in Process Improvement initiatives.

12.  Works closely with Medical Director, on-call MD and/or Executive Director to review and schedule new admissions. 

13.  Reviews cases with Manager or designee and arranges time of transfer, calling Patient Access and providing pertinent patient information.

14.  Performs psychosocials, concurrent reviews, leads groups and/or meets with patients/families in the Partial Hospital Program or on the inpatient unit when requested by management or the Medical Director.

15.  Distributes daily consults among appropriate MDs/AHPs or on-call MD.

16.  Maintains the department consult log for tracking medical floor / surgical psychiatric consult forms for completion and compilation of data.

17.  Works collaboratively with the Utilization Reviewer by assisting with authorizations that were not obtained the previous day.

18.  Performs other work related duties as assigned or requested.

·         Ability to read, write and communicate in English.

·         Masters degree in Social Work or doctorate in Psychology

·         Licensed by the state of Massachusetts as an LICSW (Licensed Independent Clinical Social Worker) or doctorate in Psychology.

·         Knowledge and utilization of crisis intervention techniques.

·         Familiarity and experience in use of the DSM V with regard to psychiatric diagnoses.

·         Knowledge of external review organizations (i.e. Managed Care, Medicare, Medicaid).

·         Experience in working with an Emergency Department team is preferred.

·         Proficient with Microsoft Excel, Word and Outlook.

·         Crisis Prevention Intervention Training must be obtained within 3 months of new position.
