Cape Cod Healthcare is the leading provider of healthcare services for residents and visitors of Cape Cod.  With more than 450 physicians, 4,700 employees, and 1,100 volunteers, Cape Cod Healthcare operates two-acute care hospitals, the Cape's leading provider of homecare and hospice services (VNA), a skilled nursing and rehabilitation facility (JML Care Center), an assisted living facility (Heritage at Falmouth), the Cape's only local laboratory service (CCHC Laboratory Services) and numerous health programs.  We are currently seeking a dedicated Clinical Social Worker to join our team at Falmouth Hospital.

 

PURPOSE OF POSITION:

Provides psycho-social assessments, crisis counseling and discharge planning to Falmouth Hospital patients and their families recognizing age specific differences in assessing the needs of the patients.

 

PRIMARY DUTIES AND RESPONSIBILITIES:

1.     Assists patients and families to adjust to illness and lifestyle changes related to illness and hospitalization.

2.     Interviews patient and family members to obtain information about home environment, family relationships, health history, and patient's personality traits.

3.     Evaluates the ongoing medical and psychosocial needs for home care planning in order to set up appropriate community services.

4.     Evaluates data gathered in terms of the medical plan of treatment, available social service programs and plans a pertinent psychosocial treatment program that will provide maximum benefit for each patient.

5.     Assists patient/family in the procurement of essential equipment to allow optimum and safe function at home.

6.     Assists patient/family in the decision making process leading to the selection of and placement in an appropriate long term care facility.

7.     Provides information and referral for continuing care and crisis intervention.

8.     Functions as a liaison on behalf of patients with appropriate community agencies.

9.     Provides services which comply with: Public Health, JCAHO, Medicaid, Medicare, HMO's and PRO regulations. Screens all admissions to the hospital to determine whether discharge planning services may be needed.

10.  Provides therapeutic short term counseling to patients and families.

11.  Communicates diagnoses and activities of daily living status to nursing home staff.

12.  Provides mental health and substance abuse assessments of Emergency Department patients, recognizing each patient's requirements relative to their age-specific needs. Works with psychiatrists/APRN in assessing and treating psychiatric patients. Makes referrals and secures placement for inpatient psychiatric care and/or outpatient mental health treatment.

13.  Assists in the coordination of child abuse and elder abuse procedures and mandated reporting.

14.  Coordinates and leads patient focused conferences with physicians, nurses and other staff when the need arises.

15.  Provides crisis intervention during covered hours.

16.  Telephone follow-up of selected discharge patients, as required by the DPH.

17.  Provides training and supervision, as appropriate, for social work students as part of their field instruction.

18.  Participates in the development of in-service programs for hospital personnel addressing the psychosocial needs of patients and families.

19.  Prepares and submits monthly statistical and quality assurance reports.

20.  Assists in fulfilling audits and other federal, state, and JCAHO requirements. 

21.  Participate in screening patients for admissions into agency programs, utilizing pre-approved admission criteria.

22.  Prepare written reports of assessment results and prepare recommendations for the level of care and treatment needs; including disposition recommendations.

23.  Evaluate patient needs and advocate the least restrictive level of care consistent with the defined patient needs.

24.  Coordinates the flow of information including the preparation of the Patient Care referral form between the hospital and the other facilities and agencies.

25.  Utilizes discretion regarding the appropriate protection and/or use of confident information.

  1. Contacts managed care representatives, third party payers, primary physicians or other relevant persons in order to provide clinical and/or demographic information required to verify insurance and/or obtain authorization for clinical services.
  2. Triages all incoming calls from either internal or external referral sources.
  3. Places calls to outside facilities when the inpatient unit or PHP does not have available beds or the patient does not meet level of care.

29.  Performs other work related duties as requested.

EDUCATION/EXPERIENCE/TRAINING:

·         Master's degree in Social Work (MSW) and State licensure (L.I.C.S.W.)

·         Minimum of three years experience in an Emergency Center setting including Crisis Intervention, Psychosocials, Referrals.

·         Minimum of three years experience working with Behavioral Health patients, ED patients and families in an Emergency Center setting.

·         Familiarity with local and state community resources and ability to access same.

·         CPI (Crisis Prevention) within 4 months of new position and must renew annually.

·         Ability to work in a demanding and sometimes dangerous environment.

·         Ability to read, write and communicate in English
