Job Details:

Cape Cod Healthcare is the leading provider of healthcare services for residents and visitors of Cape Cod.  With more than 450 physicians, 4,700 employees, and 1,100 volunteers, Cape Cod Healthcare operates two-acute care hospitals, the Cape's leading provider of homecare and hospice services (VNA), a skilled nursing and rehabilitation facility (JML Care Center), an assisted living facility (Heritage at Falmouth), the Cape's only local laboratory service (CCHC Laboratory Services) and numerous health programs.  We are currently seeking a dedicated Dietitian to join our team at Cape Cod Hospital.

 

PURPOSE OF POSITION:

            

To educate patients and/or responsible family members in physician prescribed diet modifications. To provide complete nutritional care of priority and at risk patients based on preceding nutritional assessment. To participate in in-service education of non-professional health care workers and work cooperatively with representatives from other disciplines on nutrition-related problems or projects.

             

MAJOR ACCOUNTABILITIES:

   

1. Educates patients and/or responsible family member in physician ordered diet modifications for menu selecting during hospitalization and diet management after discharge.

- Uses department approved educational materials.

- Utilizes the services of a language interpreter when necessary.

           

2. Assesses nutritional status of assigned newly admitted high-priority patients.

 

- Utilizes demographic and subjective patient information as well as daily laboratory reports.

 

3. Initiates and monitors plans for medical nutrition therapy for nutritionally compromised and high-risk patients.

 

4. Consults with Clinical Nutrition Manager and/or with Nutrition Support Dietitian regarding care plans for patients with complex needs.

 

5. Makes appropriate recommendations to prescribing physician for necessary changes in diet orders, bases on results of nutritional assessment.

 

6. Documents provided medical nutrition therapy and education in the patient's medical record, discharge/departmental files.

 

7. Upholds departmental and inter-departmental protocols regarding delivery of medical nutrition therapy.

 

8. Acts as a professional resource person for non-professional and non-clinical employees within the department.

- Participates in in-service training.

- Acts on behalf of the Clinical Nutrition Manager in his/her absence.

 

9. Communicates interdepartmentally with other health care professionals, as well as with community health agencies.

- Consults with patient's primary care nurse.

- Attends Nutritional Care Committee meetings as needed.

- Participates in Performance Improvement activities.

- Involves community health care agencies in patient's home care plan when appropriate.

         

10. Participates in reviewing, updating and reformulating the hospital's diet manual and educational materials.

 

11. Performs other job related duties as assigned.

 

EDUCATION/EXPERIENCE/TRAINING:

 

Completion of Bachelor of Science Program from an accredited undergraduate college program in foods and nutrition followed by an Acacemy of Nutrition and Dietetics (formerly ADA) approved dietetic internship.

CDR RD (formerly ADA) registration, and licensure within the Commonwealth of Massachusetts within 1 year of hire required.

Knowledge in all aspects of medical nutrition therapy. 

 Good communication skills.
