Cape Cod Healthcare is the leading provider of healthcare services for residents and visitors of Cape Cod.  With more than 450 physicians, 4,700 employees, and 1,100 volunteers, Cape Cod Healthcare operates two-acute care hospitals, the Cape's leading provider of homecare and hospice services (VNA), a skilled nursing and rehabilitation facility (JML Care Center), an assisted living facility (Heritage at Falmouth), the Cape's only local laboratory service (CCHC Laboratory Services) and numerous health programs.  We are currently seeking a dedicated Physician Extender to join our team at Cape Cod Hospital.



The Staff Physician Extender will work with the Interventional Cardiovascular Medicine Service and particularly with the Interventional Team within the Cardiovascular Division of the Department of Medicine. This Physician Extender will assist the attending cardiologist in all areas of patient care in the Interventional Cardiovascular Medicine Service. Rotation and call schedules will be determined by patient need and may include both nighttime on-call responsibilities and weekend coverage.



1.      Pre-Procedure Patient Care:

    • The Staff Physician Extender will be responsible for obtaining and coordinating the following general items: history and physical, laboratory results, charting of clinical findings, admissions orders, nursing instructions and other required elements.
    • The Staff Physician Extender, in collaboration with the Laboratory Flow Managers, will coordinate admissions and transfers to the hospital, and ensure that informed consent for procedures is obtained.
    • The Staff Physician Extender will perform pre-procedure assessments in the Pre-admission testing center, Recovery room, holding area or outpatient setting as needed.
  1. Interfaces between Cardiac Catheterization and Interventional Laboratory and Recovery Area:
    • The Staff Physician Extender will be knowledgeable about the Cardiac Catheterization and Intervention Laboratory scheduling, procedure room turnover and availability of resources. He/she will be called upon to manage patients after their procedures and while they await transfer to patient floor, and will assure availability of equipment for special procedures.
    • The Staff Physician Extender will ensure blood bank and formal or standing surgical back up when required for patient care.
    • The Staff Physician Extender will assist in family liaison.
  2. Medical Treatment and Post Procedure Care:
    • The Staff Physician Extender will be responsible for many aspects of care under the supervision of the attending physician, including but not limited to: fluid and electrolyte management, nutrition, pain management, transfusion therapy, anticoagulation management, education and post-discharge planning.
    • Following procedures, the Staff Physician Extender will manage vascular access, monitor and manage arrhythmias, pacing, drug therapy, O ? therapy, and other non-cardiac medications, under supervision of the attending physician.
    • The Staff Physician Extender will assist in the evaluation and management of clinical events such as fever, altered hemodynamics, CHF, pulmonary, complication, abrupt vessel closure, arrhythmias, access site complications and others.
    • The Staff Physician Extender will assist in transfers to and from the intensive care units.
  3. Procedure responsibilities:
    • The Staff Physician Extender will assist the attending physician during diagnostic, interventional and research procedures performed in the Cardiac Catheterization and Intervention Laboratory and other clinical areas.
  4. Discharge Planning:
    • The Staff Physician Extender will perform patient education regarding discharge including preventive care and risk factor reduction. He/she will write discharge orders, prescriptions and dictate discharge summaries in collaboration with the attending physician.
    • The Staff Physician Extender may act as a liaison between the attending Cardiologist and family members, and will schedule follow-up care as necessary, including VNA if needed.
  5. Research and Education
    • The Staff Physician Extender will be offered the opportunity to participate in aspects of research involving planning, performance, and analysis of data, and publication.
    • The Staff Physician Extender will complete 100 hours of continuing medical education necessary during each 2-year period of employment to maintain his/her PE certification Status. He/she must pass a National PE re-certification exam every six years. The PE is to be ACLS provider and is to be re-certified every two years.
    • The Staff Physician Extender will attend internal and external education meetings and read appropriate professional journals in order to maintain and enhance skills and clinical competence.
    • The Staff Physician Extender will be encouraged to advance his/her medical and tactical skills as appropriate to deliver optimum patient care.
  6. Supervisory Responsibilities
    • The Staff Physician Extender will supervise medical students (MD and/or physician assistant) who do clinical rotations in the Cardiac Catheterization Laboratory as directed by the attending physician.
    • The Staff Physician Extender will teach students, house staff, nurses, technicians and cardiology fellows as instructed by the attending physician, particularly with regard to the evaluation, interventional treatment and longitudinal care of patients with cardiovascular disease.

Physician Extenders may possess one of two educational backgrounds:

1.      Must be a graduate of a 2-year graduate level program approved by the AMA Board of Approval and Certification PA Programs.  Physician Assistant is preferred to be Board Certified by the National Commission on Certification of Physician Assistants. 

2.      Nurse Practitioner with 3+ years cardiovascular service-line experience preferred.  Massachusetts Nursing License.


·        US Department of Justice and Massachusetts' Controlled Substance DEA Registrations are required for prescribing practitioners.

·        BLS

·        ACLS

·        Must have broad knowledge of the medical environment and surgical procedures.

·        Must exercise the highest degree of professional judgment under the direction and guidance of a licensed physician/surgeon.

·        Must be able to manage and direct other clinical staff in order to carry out patient care plans.  Must leverage staffing resources and take action when clinical or professional performance does not meet the standards of care.

·        Must be able to accept instruction and guidance as assume graded responsibility within a complex teamwork framework.

·        Must be able to administer such care and treatment and exercise independent originality in problem solving not covered by existing guidelines, within legal limits, to meet the needs of the patient.

·        Must maintain a professional attitude toward patients and family members at all times and develop sound working relationships with other staff involved in patient care.

·        To maintain and enhance skill and clinical competence must attend internal and external educational meetings and read appropriate professional journals and literature.
