Cape Cod Healthcare is the leading provider of healthcare services for residents and visitors of Cape Cod.  With more than 450 physicians, 4,700 employees, and 1,100 volunteers, Cape Cod Healthcare operates two-acute care hospitals, the Cape's leading provider of homecare and hospice services (VNA), a skilled nursing and rehabilitation facility (JML Care Center), an assisted living facility (Heritage at Falmouth), the Cape's only local laboratory service (CCHC Laboratory Services) and numerous health programs.  We are currently seeking a dedicated Program Manager Advanced Care Planning and Prognostication Program to join our team at Cape Cod Healthcare.

Responsible for Planning, Coordinating, and Managing the Strategic Development of an integrated standard of practice addressing Quality of Life management throughout Cape Cod Healthcare and the Cape Cod community.




  1. Develop and implement a comprehensive Advance Illness management program in the outpatient community setting.
  2. Provide leadership of Advanced Illness management staff and oversee clinical services including clinical protocols/outcomes,    patient/family satisfaction, quality measures, physician communication and interdisciplinary team communication and palliative care best practice education.
  3. Work closely with internal and external constituents to ensure that services will be integrated with existing capabilities across transitions of care and the care continuum.
  4. Coordinate the development, implementation and evaluation of an Advance Care Planning component to Advance Illness management program as well as general community education across the care continuum.
  5. Responsible for physician outreach and education, collaboration with CCHC entities and medical groups.
  6. Coordinate the integration ACP services across care continuum facilitating consultative services related to goals of care and being an expert resource to the healthcare team.
  7. In collaboration with CCHC Coordinate community education and outreach, facilitating goals of care discussions with patients/families and changing the culture of healthcare delivery.
  8. Performs other duties as assigned.

·        Ability to read, write and communicate in English

  • Basic Computer skills
  • Good interpersonal skills, being concise by phone, email and in person
  • Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts
  • Bachelor's Degree in Nursing Required.  Master's degree preferred
  • Five years in RN clinical role in palliative/hospice care setting and Public Health experience required
  • Basic Life Support for Healthcare Provider required
  • Certified Hospice and Palliative Nurse (CHPN) and End of Life Nursing Education Consortium Certification (ELNEC) required
