Job Details:

Cape Cod Healthcare is the leading provider of healthcare services for residents and visitors of Cape Cod.  With more than 450 physicians, 4,700 employees, and 1,100 volunteers, Cape Cod Healthcare operates two-acute care hospitals, the Cape's leading provider of homecare and hospice services (VNA), a skilled nursing and rehabilitation facility (JML Care Center), an assisted living facility (Heritage at Falmouth), the Cape's only local laboratory service (CCHC Laboratory Services) and numerous health programs.  We are currently seeking a dedicated Social Worker to join our team at Cape Cod Hospital.

 

PURPOSE OF POSITION:

 

Plans, organizes and implements social work services to patients and families.  Plans for discharge and aftercare.  Maintains contact with referring agencies and related people, and with community resources used for referral.

 

PRIMARY DUTIES AND RESPONSIBILITIES:

 

1.     Assists patients and families to adjust to illness and lifestyle changes related to illness and hospitalization.

·         Interviews patient and family members to obtain information about home environment, family relationships, health history, and patient's personality traits.

·         Assists patients and families to understand, accept, and follow medical recommendations.

·         Utilizes resources to assist patient to resume life in the community or to learn to live with disability.

 

2.     Assists in patient care planning to ensure continuation of treatment in an atmosphere conducive to preservation of health. 

·         Evaluates the ongoing medical and psychosocial needs for home care      planning in order to set up appropriate community services.

·         Evaluates data gathered in terms of the medical plan of treatment, available social service programs and plans a pertinent psychosocial treatment program that will provide maximum benefit for each patient.

·         Assists patient/family in the procurement of essential equipment to allow optimum and safe function at home.

·         Assists patient/family in the decision making process leading to the      selection of and placement in an appropriate long term care facility. 

·         Performs a variety of services such as advising on social problems,      arranging for discharge or post-operative care at home or in institutions, helping to alleviate level of anxiety and fear.

3.     Provides information and referral for continuing care and crisis intervention.  

·         Brings voluntary community based services into play as appropriate to patient and family needs.

·         Utilizes resources to assist patient to resume life in the community or to learn to live with disability. 

·         Functions as a liaison on behalf of patients with appropriate community agencies.

·         Evaluates the appropriateness of community services as possible and      develop plans to meet patient needs.

 

4.     Works cooperatively with co-workers and communicates effectively with  departmental staff; other departmental personnel and health care facilities;  physicians, patients and visitors.

·         Acts in accordance with professional ethics and within departmental      policy and procedure.

·         Functions within the professional Social Work Code of Ethics. 

·         Is available to all clinical areas for social work services as needed and assigned. 

·         Prepares and updates records for each patient, documenting as per      policy.

·         Maintains monthly statistical data.

·         Observes all safety, infection and environmental control procedures.

·         Functions in such a way as to meet departmental quality assurance program.

·         Assists in care and maintenance of department equipment and supplies. 

·         Attends and actively participates in meetings and serves on committees as requested.

 

  5.  Performs other work related duties as requested/directed.

LCSW S17: 

PURPOSE OF POSITION

Plans, organizes and implements social work services to patients and families

in the emergency center.  Plans for discharge and aftercare.  Maintains

contact with referring agencies and related people, and with community

resources used for referral.

 

 

EDUCATION/EXPERIENCE/TRAINING

·         Master's degree in Social Work (MSW) or related discipline and State licensure (L.C.S.W.)

·         Significant experience in an Emergency Center setting.

·         Significant crisis intervention training and experience.

·         CPI (Crisis Prevention) within 4 months of new position and must renew annually.

·         Ability to work in a demanding and sometimes dangerous environment.

·         Familiarity with community resources and ability to access same.

·         Significant experience in working with patient and families in an Emergency Center environment.

 

LICSW S18: 

PURPOSE OF POSITION

 

Plans, organizes and implements social work services to patients and families.

Plans for discharge and aftercare.  Maintains contact with referring agencies

and related people, and with community resources used for referral.

 

EDUCATION/EXPERIENCE/TRAINING

 

·         Master's degree in Social Work (MSW) and State licensure (L.I.C.S.W.)
